A claim of any complexity accumulates a file: the loss notice, photographs, repair estimates, invoices, medical records, police reports, correspondence, coverage documents, expert reports, legal notices. They arrive over weeks, from different senders, through different channels, in no particular order.
Most of the automation attention in claims goes to extracting fields from these documents. The larger operational win usually sits one step earlier — deciding what each document is, which claim it belongs to, whether it changes anything, and who needs to see it now rather than in three days.
The three questions
Every inbound document needs three answers before its contents matter.
What is it? Type classification, from a taxonomy that reflects how your organisation actually works. Distinguishing "medical bill" from "medical record" matters because they go to different people and start different clocks.
What does it belong to? Association with the correct claim. Documents arrive with a claim number in the subject line, or in the body, or on page four, or not at all. Association by policy number plus date of loss plus insured name is a resolution problem, and getting it wrong is worse than leaving it unassociated — a document filed against the wrong claim is effectively lost, and may surface in a place it should not.
Does it change anything? The question that determines urgency. A routine invoice on an open claim is filed. A legal notice, a fatality report, a demand letter, an authority complaint or a document indicating the loss is substantially larger than reserved needs to reach a human quickly, and the routing decision is worth more than any field extracted from it.
Building the taxonomy
Type taxonomies fail in two directions. Too coarse and the routing carries no information. Too fine and the classifier degrades while nobody uses the distinctions.
A workable approach is hierarchical: a small set of top-level categories, each with subtypes, where routing decisions attach to the level that actually drives different behaviour. Misclassification within a category is then recoverable, while the consequential distinctions sit at the top.
Two categories are essential and often missing. Unknown — for documents that do not match anything, which must go to a person rather than being forced into the nearest bucket. And not claim-related — for the spam, the misdirected mail and the duplicate that would otherwise clutter the file.
Urgency, done deliberately
Urgency detection is where triage produces its clearest value, and it should be built as explicit rules over extracted signals rather than as a model judgement.
Signals worth encoding: legal or regulatory correspondence; any indication of injury severity change or fatality; documents with statutory response deadlines; amounts materially above the current reserve; complaints; documents from a regulator or ombudsman; anything referencing litigation.
Each of these is detectable with reasonable reliability and each has a defined handling path. Building them as rules means an adjuster can see why something was escalated, and a compliance function can audit the logic — both of which matter more here than marginal accuracy.
Completeness, not just presence
The most underused capability in claims document handling is checking for what has not arrived.
Given a claim type and its stage, the expected document set is largely known. A system that tracks the gap can prompt for missing items automatically, days or weeks before an adjuster would have noticed during a file review. In claims, elapsed time is cost — through cycle time, through leakage, and through customer dissatisfaction — and chasing a missing document early is one of the cheapest interventions available.
This works best as a per-claim checklist maintained continuously rather than as a report someone runs. The system knows what arrived; it should also know what is outstanding and how long it has been.
Duplicates and near-duplicates
Claim files are full of the same document arriving twice: re-sent by the insured, forwarded by a broker, included again in a later bundle. Exact-duplicate detection by content hash is trivial and worth doing at ingest.
Near-duplicates are harder and more important. A revised estimate that differs from the original in one line item, or a resubmitted form with one field changed, must not be treated as a duplicate — and must not be filed as if it were unrelated. The useful behaviour is to detect the relationship and surface the difference: this is version 2 of the estimate you already have, and the labour figure changed. That is exactly what an adjuster needs and precisely what a plain filing system cannot provide.
Extraction, scoped by type
Once type is known, extraction becomes tractable because the schema is known. Different types need different treatment:
- Estimates and invoices — line items, totals, provider, dates. Arithmetic validation applies and is highly effective.
- Medical records — dates of service, providers, diagnoses, treatment. Sensitive data with its own handling requirements; consider whether full extraction is needed or whether indexing and retrieval is sufficient.
- Correspondence — sender, date, subject, any deadlines, any commitments made. Deadlines are the high-value field.
- Photographs — metadata, and possibly damage classification. Often more useful indexed than extracted.
Note the pattern: not every document type needs deep extraction. For a large part of a claim file, correct classification, correct association and reliable retrieval deliver most of the value at a fraction of the cost.
What to measure
- Association accuracy — the share of documents attached to the right claim, and specifically the rate of wrong-claim filing, which is the serious error.
- Classification accuracy per type, with the confusion matrix.
- Urgency recall — of documents that should have been escalated, how many were? This is the metric where a false negative is expensive and a false positive is merely annoying, so tune it accordingly.
- Time to adjuster visibility — from receipt to appearing in the right person's queue.
- Unassociated backlog — documents the system could not place. A growing pile here is a leading indicator of trouble.
The realistic ambition
Claims document triage is not a workflow where full automation is the goal. The goal is that when an adjuster opens a file, everything is filed correctly, the new items are flagged, the missing items are listed, and anything urgent has already been surfaced.
That is a substantial improvement in a job that currently involves a great deal of sorting before any adjudicating begins — and it is achievable with far less risk than automating the decisions themselves.
