Insurance supervisors across major markets have converged on a broadly similar expectation: an insurer using AI in its operations should be able to explain what the system does, demonstrate that it is monitored, show that consumers are not being unfairly disadvantaged, and produce evidence rather than assurances.
Requirements differ by jurisdiction and continue to develop, so treat what follows as an operating framework rather than a compliance checklist — and confirm the specifics with your own regulatory and legal functions. The framework itself is stable, because it reflects what supervisors consistently ask for.
Start by scoping honestly
The first question is which parts of your deployment are in scope, and the answer is usually narrower than people fear and broader than they hope.
A system that extracts facts from documents — reading a date of loss, a limit, a vehicle identification number — is a data capture process. It has accuracy and control requirements, but it is not making a decision about a consumer.
A system that influences an outcome — triage that determines handling path, screening that routes to investigation, prioritisation that affects service levels — is in a different category, even when a human signs off, because the human's decision has been shaped.
The distinction is worth drawing deliberately in your documentation, because it determines the depth of governance each component needs. It is also why the architectural separation of extraction from decision, discussed elsewhere on this blog, has a governance payoff as well as a technical one: it puts most of your AI surface in the lower-scrutiny category and leaves a small, rule-based, explainable layer where the decisions are.
The inventory
Every governance framework starts with knowing what you have. For each AI component:
- What it does, in one paragraph a non-specialist can follow.
- Where it sits in the process and what depends on its output.
- What data it consumes and what it produces.
- Whether a human reviews its output, and how the review works in practice.
- Who owns it, who approved it, and when it was last assessed.
- The vendor, if any, and the terms governing model changes.
That last item catches a common gap: if your vendor can change the underlying model without notice, your inventory is describing something that no longer exists. Contract for notification of material model changes and record the version in use.
Documentation the review will ask for
Purpose and scope. What decisions this system informs and, explicitly, what it does not do.
Data. Sources, fields, sensitive categories, retention, and where it is processed and stored. If personal or health data is involved, the lawful basis and the handling controls.
Performance. Accuracy against a labelled corpus, broken out per field, with the methodology stated — including how ground truth was established and what normalisation rules applied. A single headline number will not satisfy a serious review.
Limitations. Where the system performs worse: document types, quality bands, languages, formats. Stating limitations plainly builds far more credibility with a reviewer than claiming uniform performance.
Human oversight. What is reviewed, at what threshold, by whom, with what training, and what a reviewer can override. Include the evidence that reviewers actually exercise judgement rather than rubber-stamping — reviewer correction rates are the useful artefact here.
Monitoring. What is tracked in production, at what frequency, what the thresholds are, and who receives the alerts.
Change control. How prompts, models, rules and thresholds are versioned, tested and approved. The regression testing practice described elsewhere on this blog is precisely the evidence this section needs.
Incidents. What has gone wrong, what was done, what changed as a result. An empty incident log on a system running at scale reads as a monitoring failure, not a quality achievement.
Fairness, applied to document processing
Fairness analysis in document AI is less obvious than in pricing models, but it is not absent, and supervisors have begun asking about it.
The realistic concerns are about differential performance rather than explicit discrimination. Extraction accuracy can vary systematically with document characteristics that correlate with protected or sensitive attributes: handwritten submissions versus digitally generated ones, non-English documents, older or lower-quality scans, submissions from particular channels or geographies.
If handwritten claims take longer because they route to review more often, that is a service disparity with a plausible correlation to who submits handwritten forms. It may be entirely defensible — but you should know it is happening and be able to explain it, rather than learn about it from an examiner.
The practical control: segment your accuracy and cycle-time metrics by document characteristics, review the segments periodically, and document what you found. That is a modest amount of analysis and it converts an unknown into a managed risk.
Monitoring that actually runs
Governance documentation describing monitoring that nobody performs is worse than no documentation. Keep the production monitor small enough to be sustainable:
- Field-level accuracy on a rolling audit sample.
- Straight-through rate and escaped error rate.
- Reviewer correction rate and override rate.
- Field null-rate and value distributions, to catch drift.
- Volume and type mix, to catch input changes.
- Segment breakdowns as above.
Set thresholds that trigger a review, name the person who receives them, and record what happened. The record of alerts investigated is one of the most persuasive artefacts you can present, because it demonstrates the framework operating rather than existing.
Vendor obligations to negotiate
If you are buying rather than building, the governance requirements flow through to the contract:
- Notification before material model changes, with a window to test.
- Access to per-field performance data on your own documents.
- Audit rights, security attestations, and the subprocessor list.
- Data handling, residency, retention and deletion commitments.
- Support for your own audit sampling and evaluation.
- Data portability and exit terms agreed at signing.
A vendor that treats these as unusual requests is a signal in itself. In regulated insurance, they are ordinary.
The underlying point
The governance work described here overlaps almost entirely with the engineering practices that make a document AI system good: measure per field, keep a labelled corpus, version your changes, monitor in production, define human oversight, know your limitations.
Teams that build well have most of the governance package as a byproduct. Teams that treat governance as a documentation exercise bolted on at the end usually discover that the artefacts do not exist because the practices did not either.
