Two categories of clinical voice product are frequently compared as though they were alternatives: ambient documentation, which listens to a clinical encounter and drafts a note, and structured dictation, where a clinician deliberately dictates into a defined report format.

They are not the same product with different interfaces. They solve different problems, fail differently, and suit different specialties. Evaluating them against a single set of criteria produces a poor decision in both directions.

Ambient documentation

A microphone captures the clinician-patient conversation. The system produces a draft note.

What it is good at. Removing documentation from the encounter entirely. The clinician gives full attention to the patient and reviews a draft afterwards. In specialties where the note is largely a narrative record of a conversation — primary care, many outpatient consultations, behavioural health — this is a substantial improvement in both efficiency and the quality of the interaction.

Where it is hard. The source is conversational speech: interruptions, overlapping speakers, ambient noise, digressions, family members contributing, and a great deal of content that does not belong in the note. The system must decide what is clinically relevant, which is a judgement, not a transcription task.

The characteristic failure. Plausible content that was not said, or that was said by the wrong person, or that reverses a nuance. A patient saying "I stopped taking it because it made me dizzy" and a note recording "patient reports medication intolerance" is a reasonable inference. A note recording "patient continues on medication" is a fabrication that reads perfectly naturally. Because ambient notes are fluent by construction, errors do not announce themselves.

Oversight implication. Review must be genuine, and the interface should make it easy to check the draft against what was said. Clinicians reviewing fluent, plausible drafts under time pressure will approve them; that is a predictable human behaviour, not a training problem, and the system design has to account for it.

Structured dictation

The clinician dictates deliberately into a report structure — pathology, radiology, operative notes, procedural reports.

What it is good at. Precision. The clinician controls exactly what enters the record, in the order and structure required. Domain vocabulary can be modelled tightly because the content domain is narrow. Output can be structured, validated and made machine-usable.

Where it is hard. Specialised terminology, measurements, verbal navigation and self-correction, and the fact that a single misheard term can invert a clinical meaning. Covered in more depth in our post on pathology dictation.

The characteristic failure. A substituted term. Less likely than an ambient fabrication, and more consequential when it occurs, because these reports often are the diagnosis rather than a record of a conversation.

Oversight implication. Term-level confidence surfaced in the editor, so review attention goes where uncertainty is. The clinician is reviewing their own dictation, which is a much easier verification task than checking a generated note against a remembered conversation.

The comparison that matters

The asymmetry in error visibility is the most important row. In dictation, the clinician knows what they said, so a discrepancy is detectable. In ambient capture, the clinician is reconstructing a conversation from memory while reading a fluent draft — a materially harder verification task.

Choosing

Ambient is the stronger fit where the encounter is a conversation, the note is narrative, documentation burden is a genuine driver of clinician dissatisfaction, and content is descriptive rather than determinative.

Structured dictation is the stronger fit where output is a formal report, terminology is specialised, measurements and structured values matter, downstream systems consume the fields, and the report is itself the clinical product.

Many organisations need both, in different departments. Procuring one enterprise-wide because it is simpler contractually is how a radiology or pathology department ends up with a tool that does not fit its work.

Questions worth asking either vendor

  • How is the output grounded, and can a reviewer trace any statement back to the source audio at the relevant moment?
  • What is the measured rate of content in the draft that was not said? Ask for the methodology, not a number.
  • How is negation handled, and can you show the specific test results?
  • What is the per-clinician accuracy distribution, not the average?
  • What happens to audio — retention, access, deletion — and is it covered by the BAA?
  • How does the system behave when it is uncertain? Does it flag, hedge, or omit?
  • What is the measured edit distance between draft and signed note?

That last metric is the most honest single number either category can offer, because it captures everything the clinician actually had to fix.

The shared constraint

Both categories produce drafts. Both require a clinician to review and sign. Both put PHI into audio, transcripts and logs, with the handling obligations that follow.

And both are evaluated best not on a demo but on a fortnight of real use by the clinicians who will live with the output — including the ones with accents, the ones who talk fast, and the ones who were sceptical. Those are the people whose experience determines whether a deployment succeeds, and they are systematically absent from vendor pilots.
